Clinical competency forms may be submitted with your online application or sent to the National Office by your school/program director.

Official school transcripts must still be submitted directly to the National Office by your school or an authorized third party source.

Be sure to review all standard eligibility requirements for the Perfusion Basic Science Examination (PBSE) and the Clinical Applications in Perfusion Examination (CAPE).
